The episode starts with Mahone pointing his gun at Michael and Lincoln, and the viewers wondering how the brothers will get out of this alive. Border control comes and catch Mahone red handed, and force him to put his gun down.
Sucre and the pilot make a desperate getaway from the jet behind them that will take them down if they do not escape from the plane.
Kim gets nervous when his boss wants to see him and the brothers are still breathing.
A man in a war veteran bar insults T-Bag, and as punishment, T-Bag chooses his hand to reattach to his stump.
The new warden of Fox River is interviewed, and stresses that the rules have changed, and that when the brothers return, Lincoln will be executed as early as next week and Michael will spend the rest of his life behind bars.
Michael and Lincoln are safe in a cell in New Mexico, but not for long, as Mahone is anxious to be put into the transport vehicle with them. Kim contacts Mahone and threatens his family if Michael and Lincoln reach Fox River in one piece.
The boys are denied a phone call, which they needed to get in touch with Sara, and Michael looks on hopelessly as Sara calls his phone and leaves a heartbreaking message, telling him that she didn’t leave him of her own accord and that she needs him. Shortly afterwards, Sara sees them on the television and learns of their apprehension.
T-Bag woos an unsuspecting and lonely postal worker named Denise, but their short relationship comes to a sudden stop when she realizes who he is, and T-Bag disposes of her, but not before she gave him the forwarding address of Susan Hollander.
A bemused Bellick is denied bail at his trial, and with great pressure from his attorney, accepts a guilty plea of 25 years, where he requests to be incarcerated in Fox River. Former Captain Brad Bellick gets the shock of his life when he finds out about the new tough laws and is locked in the same cell as Avocado, ironically where he placed Tweener for punishment.
Mahone makes a final attempt to get placed into the same transport van as Michael and Lincoln but fails with Michael looking at Mahone, knowing that something is up.
Kellerman calls Kim in an apparent attempt to get his job and life back. He makes a deal with Kim and is set to cause a commotion in Albuquerque and Kim contacts Mahone and forces him to put Kellerman at the top of his hit list. We finally meet Kim’s boss, but we are still left in the dark about who he really is and why he writes orders down for Kim.
Kellerman has his people leave the van unlocked, and the keys inside with Lincoln and Michael in attempt to lure them out.
A devastated Sara is seen in the bathroom with a razor, which we later find out that she uses to cut her hair in an attempt to disguise herself.
Sucre makes the jump out of the plane, but the pilot received the faulty parachute and dies. Sucre is last seen walking in the Sonoran Desert, 40 miles south of the US/Mexico border.
T-Bag finally finds Susan Hollander, and lets himself in as she stares at him in horror, ending with the words, “don’t be frightened, Teddy’s home.”
After much deliberation, Michael and Lincoln make their getaway, narrowly missing the gunshots. Mahone takes his own route to find the brothers, and he and Kellerman have them boxed in. Kellerman and Mahone both have the brothers at gun point, when Kellerman then points his gun at Mahone and shoots him in the shoulder. He then tells the stunned brothers that if they want his help they need to follow him now.
